Job Description

To lead on the brokerage of services on behalf of residents who have specific social care needs, using expert knowledge of client groups, systems, care markets and negotiation skills to ensure that appropriate, value for money care is commissioned. To meet the new NHSE Discharge Guidance to support hospital discharges to community settings operating within the Discharge to Assess model and working the extended hours of delivery. To work unsocialable hours Monday to Friday inclusive 4pm to 8pm and Saturday and Sunday 8am to 4pm

Provide advice and make recommendations based on up to date knowledge and analysis / evaluation of information.

Manage escalated or complex customer issues within the relevant area.

Research developments in relevant area. Collate process and analyse information / data. Translate outputs into advisory reports / documents / actions as appropriate.
